HOW TO REFER TO THERAPYTOWN
1. Fax Referrals to 918.250.9976
2. We call the patient, schedule an evaluation. If we are unable to get a hold of the patient or the parent declines the referral, we send you an update.
3. Once the evaluation is complete, we will send you a compy for your medical records.
GENERAL RULES FOR REFERRALS
Scripts are valid for 365 days for therapy if “evaluate and treat for speech/occupational/physical therapy for 12 months” is written
Scripts are valid for 90 days for evaluations
Scripts must include:
- Patient Name
- Provider Signature
- Date
- Diagnosis Code
- “Evaluate and treat for speech/occupational/physical therapy for 12 months”
- Must include all office/doctors’ notes with reason for referral.
